I give each clinician copyright permission so that you may brand the forms with your own logo and practice information. This packet is for your use only, you may not copy, reproduce, sell or share this information with other therapists.
Editing Information & Support
As an additional support, I have kept my Growth Counseling Services logo as a place marker at the top of each page, with additional highlighted information as place markers throughout the documents so that you can clearly see where to add your own information. Simply open up the header footer and copy and paste your logo and practice information to brand the forms as your own. As you read through the documents, wherever you see a GCS place marker highlighted, this is your reminder to edit with your own name or practice information. Therapists and colleagues have shared that they greatly appreciate these highlighted “flags” as a helpful reminder of where to personalize the documents.

Format: The Clinical Therapy Forms for Clinical Professionals e-packet is created in Microsoft Word and delivered via email in a downloadable zip file containing forms that you may customize. Once purchased, you must download the forms from your laptop or tablet (not a mobile device) within 24 hours. Once downloaded, you may then “brand” the forms with your own logo and practice name and other information throughout the document without the worry of copyright infringement. It is up to each therapist to stay abreast of legal and ethical requirements per their state board, thus it is your responsibility to keep your forms updated over the years per your state or region’s licensing board. While this packet is comprehensive, you may need to add additional information as required from your specific licensing board.

Every state and region/country has their own clinical governing state board. Though many states are similar in what they legally require or ethically suggest for the therapist to include in their clinical intake and client consent forms, there may be occasions where legal and ethical guidelines differ per state with respect to clinical forms. Please check with your state or region’s legal and ethical guidelines to make sure that you are adhering to your state or region’s requirements for clinical forms, and then adjust or edit as recommended by your state, region or attorney. Additionally, each purchaser/therapist is responsible for staying abreast of legal or ethical changes as mandated by their state or region’s clinical board and laws , and updating your forms as needed per your state and as your board mandates. By purchasing these forms you understand that you may need to add additional information or edit as required by your specific licensing board.
The forms are created and sent in Microsoft Word for easy editing. Mari has given each clinician copyright permission to brand the forms with your own logo and practice information. As an additional support, Mari has left her own logo and practice information highlighted in yellow as a place maker and as a road map for you to easily follow when adding your own information.
If you use an online EHR system (Electronic Health Notes), please check with the platform for directions in uploading the forms as Mari does not provide technical advise. Other therapists have shared that they have been able to upload to their EHR without issues, however, it is always wise to check with your specific EHR as each platform may vary.
