Coaching Forms Packet
I created this coaching materials packet as a support for other professionals who coach but are doing so without coaching client consent forms, and coaching worksheet materials. These well meaning professionals are scheduling coaching calls without any intake information, a process of tracking progress on their client’s goals, a means of taking payment, or clearly stated coaching policies and boundaries that are signed off on by the clients.
In doing so, they were leaving themselves open to liability, as well as unintentionally not giving their coaching clients a focused and guided process. We all start somewhere as coaches, but the most successful coaches have a solid and consistent foundation to work from in supporting their clients. I have outlined the forms I have used for years in my own practice as a support on the following page.
Each document template is formatted in Microsoft Word for ease of use. Documents included in this packet are: The Coaching 360 Form, The Coaching Consent Form, The Coaching Session Progress Report Form, The Coaching Credit Card Form/Payment Agreement.
